Sections 3, 4 & 5 (below) of the application should be no more than ten pages in total length (exclusive of citations) and should in general conform to the format of Sections A-D of PHS Form #398.
The information described below should be provided.
- A brief biographical statement with respect to the investigator should be provided. This statement should not be a restatement in narrative form of the individual’s curriculum vitae. The Foundation is interested in answers to such questions as to why the individual has elected to pursue a career in academic research, what kinds of events have impacted the individual’s life and led to his/her current career aspirations, what kind of impact does the individual hope to make in the individual’s particular field, were there important mentors who helped shape the individual’s life (and if so, who were those mentors) and what are the individual’s special attributes. There are no limitations as to discussing these types of questions in this section of the application. Members of the Foundation’s Medical Advisory Committee want to gain an additional perspective of the individual other than that which may be gleaned from a review of his/her proposed research plan.
- A curriculum vitae with respect to the particular individual should include relevant details as to degrees attained, educational and research training, teaching experience, honors and publications.
- A short (no more than 100 words) abstract in layman’s terms describing the proposed research should be provided.

This statement should include a discussion noting the potential impact of the proposed research on the existing body of knowledge in the particular field or in allied fields and how the proposed research will contribute to or extend information already known. There should also be a discussion as to how the proposed investigation relates to new directions with respect to resolving or understanding pathophysiology, therapeutics or other medically related problems.
- Because this grant is a Career Development Award, it is important that the application contain a description of a realistic program for additional relevant training, which would advance the individual’s long-term career in academic research. Such additional training may be at institutions in the United States or at foreign centers as appropriate.
- No final decision will be made as to any application involving human experimentation of any kind, including work with specimens such as human blood samples, unless the Foundation receives appropriate documentation with respect to Institutional Review Board Approval and copies of approved Informed Consent Forms. Similarly, no final decision will be made as to any application involving the use of laboratory animals until the Foundation receives appropriate documentation indicating compliance with applicable laws and accepted procedures relating to the humane treatment of laboratory animals.
